Case ManagementCase Management is voluntary and is designed to be a supportive and complimentary service to the behavioral health services we currently offer our patients. It includes regular check-ins with each participant to assess and evaluate the client’s progress towards goals in areas such as basic life skills, employment, support services, and access to other community resources.
We realize that people sometimes struggle with finding and coordinating the community resources they need. They often require assistance in understanding the various supports and options that are available to them as well as help in navigating the process of obtaining those supports. Our team of professional Case Managers works with individuals and families to help them identify and obtain community resources that will increase their individual stability and overall wellness. We empower individuals to find the community support they require to ensure their basic needs can be met. Services may include legal assistance, food assistance, and assistance with housing. What can my Case Manager do for me?Although your behavioral health case manager will not provide counseling or mental health treatment services, they may do any or all of the following:

Peer SupportPeer support empowers people to make the best decisions for them and to strive towards their goals in their communities. Peers are an essential component of recovery-focused systems and are key across settings and stages of recovery.
Peer support is an opportunity to work with someone who has a lived experience with recovery in mental health, substance use, or trauma related challenges. A Peer Support Specialist can understand, empathize and support you in your recovery process.
The goal of peer support is to help you find and build on your strengths and skills to overcome your barriers.
