Your Own Personal Health Coach To Help You Reach Your Health Goals

In the Care Management program you will be assigned a Care Manager who will act as your own personal health coach. Your Care Manager will get to know you and your needs in depth and discover new ways to help you successfully achieve a more healthy lifestyle. This relationship will be focused on learning about you as an individual, your life and health goals, past treatment experiences and preferences, your strengths and weaknesses, and any other health barriers you face.

The choice to change your health is in your hands, and your Kaniksu Community Health Care Manager is here to help make the process easier for you.
What a Care Manage can do for you: 
  • Create a personal care plan to guide you towards set goals
  • Provide follow-up and feedback to enable your success
  • Keep you educated and informed about your health while teaching you the skills needed to self manage your health
  • Help you to better understand your specific barriers to improved health, and guide you in making a change
  • Help to connect you with community resources around your specific needs, including but not limited to: support groups, specialists, financial & transportation services. child or elderly care services, etc.
  • Coordinate all of your care providers, specialists, behavioral health, and community resources to streamline your overall experience
  • Act as your Kaniksu champion who will go above and beyond to help you reach your health goals

The Process: 
  • Introduction to your Care Management Program:
    • ​Your Primary Care Provider (PCP) will visit with you and organize a "meet and greet" between you and your Care Manager.
    • You will talk with your Care Manager at this time about when is best to meet and begin the program.
  • Your Care Manager  will schedule a 45-60 minute appointment for care planning:
    • The appointment will include you, and any other relevant family members or care takers.
    • Your care plan is developed, goals are stated and barriers assessed.
    • Relevant information and resources will be provided.
    • Your Care Manager will coordinate any needed visits with external service providers.
  • Your Care Manager will work with your PCP on your care plan, and follow-up with you:
    • Your Care Manager will provide a care plan to both you and your PCP.  The plan will be monitored by the Care Manager to promote success.
    • Your Care Manager will discuss any further developments and progress with your PCP and decide on the next step(s) in the care plan process.
    • They will then schedule the next appointment with you to talk about progress made and the next step.
    • Follow-up appointments will be done at the same time as your regular PCP appointments for your convenience.

Have an urgent healthcare question after hours? A triage nurse is available 24/7 for your healthcare needs by calling (208) 263-7101.
Should a provider consult be necessary, the after hours triage nurse has access to an on-call KCH provider. Language services are available.  Medically trained interpreters are available via phone to assist patients with their healthcare questions.

Kaniksu Community Health an FTCA Deemed facility.  This health center receives Health and Human Services (HHS) funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health related claims, including medical/dental/behavioral health malpractice claims, for itself and its covered individuals​.
The Federally Supported Health Centers Assistance Act of 1992 and 1995 granted medical malpractice liability protection through the Federal Tort Claims Act (FTCA) to HRSA-supported health centers.
