​If you are a new patient, call for a “new patient appointment,” and we’ll help you choose a provider that meets your needs.  At KCH we are committed to being your Patient Centered Medical Home and are committed to providing evidence-based care to all our patients and families to support self-management and coordination of care.  We encourage you to watch the short video below to learn more about KCH and how we can best meet your healthcare needs.

Planning for your appointment will help you get the best care.

• Gather your personal information
  • Picture ID & insurance card(s)
  • Medical/Dental records, including immunization records, past medical history, family history, social history, recent labs or procedures, recent ER or urgent care visits and dates of recent hospitalizations 
  • Are you a foster parent or grandparent?  Please bring guardian or custody papers showing custody of the child/children
  • Are you an adult guardian?  Please bring Power of Attorney
• Arrive 15 minutes early
• Bring your medications, or a list of medications 
• Make a list of concerns to discuss - This will help make sure all your medial issues are addressed.
• Don’t be afraid to ask questions - Your health care team will answer questions, make follow-up appointments, and help with referrals.​

Every patient has both rights and responsibilities related to receiving health care at Kaniksu Community Health. 
Kaniksu values and protects your privacy and confidentiality.  For more information, see our privacy policy.  All employees of Kaniksu are bound by the privacy protections of the federal known as HIPPA (link), the Health and Insurance Portability Accountability Act.

Report concerns of noncompliance at any KCH clinic, or phone us at (208) 263-7101 and a member of our staff will assist you.

Medicare Shared Savings Program
KCH participates in the Medicare Shared Savings Program ACO. ACOs are organizations formed by groups of doctors and other health care providers that pledge to high quality, patient-centered, coordinated care for patients covered by Medicare.
ACOs must meet quality standards to ensure that savings are achieved through improving care coordination and providing care that is appropriate, safe and timely. CMS has established quality measures on care coordination and patient safety, appropriate use of preventive health services, improved care for at-risk populations, and patient and caregiver experience of care.
